Staff Roles

Here are the responsibilities of each type of staff member:

  • Registered Nurse (RN)

Registered Nurse (RN)

  • Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Develops and/or follows an individualized Plan of Care and update the Care Plan
  • Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period.
  • Assessment skills as applied to the client, family/support system, and environment.
  • Teaches skills according to the clients needs and within the context of the client’s environment, value system, and physical/emotional status.
  • Medication administrations.
  • Practices nursing with respect for individual, cultural, and spiritual differences.
  • Maintains confidentiality in all aspects of the job.
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Assesses environmental safety and takes initiative to prevent accidents and promote safety.

Licensed Practical/Vocational Nurse (LPN)

  • Provides professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
  • Scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration and monitoring, client teaching, emergency, intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Maintains confidentiality in all aspects of the job.

Home Health Aide

  • Range of motion exercises.
  • Assistance in ambulation or exercises.
  • Bathing
  • Shampooing
  • Skin care/nail care
  • Oral hygiene
  • Shaving
  • Dressing
  • Meal preparation/feeding
  • Laundry
  • Light housekeeping
  • Maintain confidentiality in all aspects of the job.

Homemaker

  • Functions within the limitation of the individual assignment.
  • Provides companionship, socialization, and housekeeping services.
  • Assists with meal planning, purchase of food, and food preparation.
  • Assists clients to live in clean, safe environment:
    • Cleans bathrooms and kitchen, including stove and refrigerator.
    • Personal errands, per client and Agency request.
    • Laundry.
    • Vacuuming and dusting.
    • Scrubbing floors.
    • Changing bed linens.
  • Treats clients and families with respect. Maintains confidentiality.

Physical Therapist

  • Performs assessment of client’s status including strength, balance, and environmental parameters
  • Develops and/or follows an individual plan of care and updates the plan of care
  • Provides skilled interventions with a focus on achieving realistic client outcomes within a specified time
  • Teaches skills according to the client’s needs and within the content of the client’s environment, value system, physical/emotional status
  • Practices physical therapy with respect for individual, cultural, and spiritual differences
  • Maintains confidentiality in all aspects of the job

Physical Therapist Assistant

  • Provides professional physical therapy care as defined in the physical therapist plan of care under the direction of the physical therapist
  • Provides physical therapy visits for clients as directed by the physical therapist
  • Follows the care plan established by the physical therapist
  • Scope of clinical practice includes monitoring reporting findings, following care plans, client safety teaching, emergency, intervention, teaching endurance, strength, and balance techniques according to plan of care

Occupational Therapist

Occupational Therapist Assistant

  • Provides professional occupational therapy care as defined in the occupational therapist plan of care under the direction of the occupational therapist
  • Provides occupational therapy visits for clients as directed by the occupational therapist
  • Follows the care plan established by the occupational therapist
  • Scope of clinical practice includes personal care services, monitoring reporting findings, following care plans, client safety teaching, emergency, intervention, teaching endurance, strength, and balance techniques according to plan of care
  • Promotes personal safety and a safe environment for client and coworkers.
  • Maintains confidentiality in all aspects of the job

Social Services

  • Performs comprehensive assessment of client’s psychosocial status and environmental parameters
  • Develops an individual plan of care and update the plan of care
  • Assesses skills as applied to the client, family/support system, environment
  • Teaches skills according to the client’s needs and within the content of the client’s environment, value system, physical/emotional status
  • Connects client with available resources
  • Support system for the client
